PHMC Health Center at Temple is a Federally Qualified Health Center that delivers full access to patient centered care, in a warm and welcoming setting. We have a bilingual staff and interpretation services available. Our health center team has received recognition as a Patient-Centered Medical Home and takes pride in providing world class care for the families in the community we serve.

Our health care services include:

  • Adult and pediatric primary care
  • Family planning services
  • Women’s healthcare services
  • Immunizations and flu shots
  • Management of chronic health conditions including diabetes, hypertension, asthma, and others
  • Behavioral health and trauma informed consultation services
  • HIV and Hepatitis C testing, counseling and treatment
  • Addiction medication
  • Health and nutrition counseling
  • Clinical social workers
  • Medical case management
  • Peer recovery services, referral navigation, health insurance assistance
  • Social services assistance (SNAP, housing, insurance, utilities, legal etc.)

PHMC Health Connection offers a pharmacy discount program. Most insurance plans are accepted and a sliding fee scale is available for uninsured patients. We see everyone regardless of their ability to pay.

Enter the health center on Tioga Street using the outpatient entrance. We are located inside of Temple Hospital next to the café on the first floor.
Broad Street Line – Erie Station stop, walk 2 blocks south on Broad Street to Tioga Street
